This Privacy Policy applies to the services offered by Lumina Laser Clinic.

- Your name and date of birth;
- contact details and your emergency contact details;
- bank account or credit card details;
- payment history (of payments to Lumina Laser Clinic)
- treatment details;
- before and after photographs;
- medical information (e.g. details of any allergies you may have).
Collection of your personal information
We will not collect any personal information about you without your consent, unless we are required or authorized by law to do so.
We collect your personal information primarily to enable us to provide you with our services and products. We may also use the information to contact you, for marketing purposes, or for any other purpose permitted under the Privacy Act.
You do not need to identify yourself when you deal with us (e.g. to find out more about treatments or products) however in certain situations we will only be able to deal with you if you have identified yourself to us.
We will collect your personal information via:
- a ‘client card’ and or consent form you will need to complete when attending the clinic for a consultation or treatments You are not obliged to provide your personal information to us, however if you do not provide your personal information, we may be unable to provide services or products to you.
Use and disclosure of your personal information
Your personal information will primarily be used by us to:
- deliver services and products to you;
- administer the delivery of such services and products;
- contact you, including to provide information to you about products, services and/or special offers:
- obtain feedback from you about the products and services we deliver to you; and
- record statistical data for marketing analysis.
Access , amendment of your personal information
- You have the right to access personal information that we hold about you. To access your personal information, please contact us in writing or contact the Clinic.
- You also have the right to ensure that the personal information we hold about you is correct, up-to-date and complete. If you would like to correct the personal information that we hold about you, please contact us in writing or contact the Clinic.
